Good day. Please send your request to enquiry.nctvet@moey.gov.jm. You will need to fill and download the certificate amendment form found on the website (‘Resources’, then click ‘applications forms’). You will also be required to send back the old certificate. You will be advised further by a customer service rep when you send the email.
